Checked your Therapeutic Use Exemption (TUE) status using the TUE Wizard?
Downloaded and completed a TUE application form (PDF 605KB)?
Gathered the supporting evidence outlined in the TUE checklist?
You must submit medical evidence with this application. Your evidence must include:
a comprehensive medical history
the results of all relevant exams, lab investigations and imaging studies
if a permitted medication can be used to treat the medical condition, provide justification for the therapeutic use exemption for the prohibited medication.
We recommend you also provide:
copies of original reports or letters where possible
a short summary including the diagnosis, key elements of the clinical exams, medical tests and the treatment plan.
